Our Social Worker works closely with the patient and family to resolve such difficulties whenever possible. Your VNA Health Home Health Care Team works with your physician to build a customized care plan to help you recover and regain your independence. Our Home Health Aides provide personal care (bathing, shampooing, simple dressings, etc.) and may perform some light housekeeping for patients who are unable to do so. Home health aides provide non-nursing services like bathing, personal hygiene, therapeutic care and light housekeeping. Provides functional supervision of all Home Health/Home Care Staff on weekends .

Hospice care may also be provided in a short-term inpatient setting, such as our hospice residence. Our team partners with patients, their doctor, family and caregivers to help home bound individuals in need of skilled medical services during recovery from illness, injury or surgery. In addition to managing chronic health conditions, our team specializes in Diabetes, COPD and Congestive Heart Failure care. PBH&HH provides education, fall prevention, care for high-risk pregnant women, infants and children, rehabilitation or orthopedic services, wound care, IV care and Ostomy management. How is a plan of care created?
Working with the patient’s physician, getting well at home is possible with an interdisciplinary home health care team of visiting nurses, therapists, caregivers and social workers. VNA Hospice & Palliative Care is a non-profit provider of comprehensive care and support to patients and families facing the challenges of life-limiting illness and loss. Care is coordinated among a team of clinicians including medical doctors, nurses, hospice aides, social workers, chaplains and trained volunteers. Support can take many forms; medical, physical, emotional, practical and spiritual, ensuring help is provided to all involved. Many patients receive hospice care in their homes, whether that's a private home or apartment, or in a nursing home or skilled nursing facility.
Licensed Physical Therapists will, upon request of the physician, assess a patient’s need for therapy. They work closely with the physician and other home health staff, as well as the patient, to develop a plan of treatment suited to the patient’s needs. Our team serves those with or without family support and those who may have limited financial resources. With the assistance of United Way of the Midlands, we are able to care for individuals who are uninsured or otherwise unable to afford or access services.

Certified home health care services are ordered by a physician, require skilled care, and is typically paid for by Medicare or other health insurances plans. VNA Care’s home health care services allow some people to come home from the hospital sooner, avoid stays in health care facilities, or learn to manage a newly diagnosed illness or disease. Our staff of Registered Nurses and Licensed Practical Nurses provide each patient with “personalized” care. Patients may be visited by the nurse as frequently as twice a day or as seldom as once a month, depending on the patient’s physical condition and the physician’s orders. The VNA’s trusted caregivers help with everyday tasks like providing transportation, setting up medication reminders and assisting with around-the-house responsibilities. VNA caregivers are insured and undergo background screenings to preserve the safety of your loved one.
