Physical Therapists assess your muscle strength, joint movement, balance and mobility and provide exercise programs and instruction to restore and enhance independence and safety. We have the experience and expertise for all your care needs. A Home Health nurse is available by phone 24 hours a day, 7 days a week. Occupational Therapists provide instruction and training to help you improve or regain independence with daily activities such as bathing and dressing.
Hospice care may also be provided in a short-term inpatient setting, such as our hospice residence. Our team partners with patients, their doctor, family and caregivers to help home bound individuals in need of skilled medical services during recovery from illness, injury or surgery. In addition to managing chronic health conditions, our team specializes in Diabetes, COPD and Congestive Heart Failure care. PBH&HH provides education, fall prevention, care for high-risk pregnant women, infants and children, rehabilitation or orthopedic services, wound care, IV care and Ostomy management. This position requires a minimum of one year experience in acute care or community setting. All of our services are provided in the comfort and privacy of your own home. I understand that if selected for an internship position, I will be required to undergo a criminal background check, physical examination, TB Gold, and drug screening. Certified home health care services are ordered by a physician, require skilled care, and is typically paid for by Medicare or other health insurances plans. VNA Care’s home health care services allow some people to come home from the hospital sooner, avoid stays in health care facilities, or learn to manage a newly diagnosed illness or disease. Our staff of Registered Nurses and Licensed Practical Nurses provide each patient with “personalized” care. Patients may be visited by the nurse as frequently as twice a day or as seldom as once a month, depending on the patient’s physical condition and the physician’s orders. The VNA’s trusted caregivers help with everyday tasks like providing transportation, setting up medication reminders and assisting with around-the-house responsibilities. VNA caregivers are insured and undergo background screenings to preserve the safety of your loved one.
A newsletter from PBHHH arrived in my email announcing training for hospice volunteers. My family had experienced the need for hospice twice, once in Ohio and again in Florida. The relief my family and I felt when the hospice volunteers arrived was beyond measure. PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.

This care is in conjunction with active treatment for a disease. The palliative care team focuses on the pain and discomfort a patient may feel while undergoing treatment while your primary doctor and specialist focus on treating your illness. While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health since October 2021. When I retired, I began searching for volunteer work that would allow me to give back to my community.

At Home Private Care, part of VNA Care, offers a wide array of supportive services that allow many people to remain independent, comfortable, and safe at home. We are currently looking for a Home Health Aide to join our team on a per diem basis to provide exceptional skilled personal care to homecare patients in Palm Beach County. Home Health Aides are companions that deliver assistance with daily living such as bathing, grooming, cooking, and light housekeeping.
Most insurances, including Medicare, provide these services at no-cost to the patient or family. Social Service is provided for patients whose recovery from illness is being impeded by social, emotional, or financial difficulties. Our Social Worker works closely with the patient and family to resolve such difficulties whenever possible. Nurses provide direct care; manage, observe, and evaluate a patient’s care; and teach the patient and his or her family caregiver. A rating if 4-5 stars would mean the agency performed better than others regarding care practices and outcome.
